Annual Physical
  • Sharing for one annual physical and limited labs is available to prioritize preventive health care. Eligible for sharing after AHP (Annual Household Portion) has been met.
  • Two routine lab tests are eligible for sharing as part of the annual physical: 
    • Basic Lipid Panel (cholesterol screening) 
    • HbA1C (diabetes screening).
  • Additional tests are not eligible for sharing unless they are for a diagnostic evaluation. 
  • Well-Child Care: Sharing for routine well-child care is eligible until the child reaches the age of six. Eligibility extends to routine check-ups and associated lab work, excluding vaccinations and/or immunizations.
